NextLevel Health Partners, Inc., Announces Partnership With PatientPing To Improve Member Care And Coordination

CHICAGO, Jan. 17, 2019 /PRNewswire/ -- NextLevel Health Partners, Inc., a Medicaid managed care company in Illinois, today announced an innovative partnership with PatientPing, a health technology company that connects providers to seamlessly coordinate patient care. PatientPing will alert NextLevel Health coordinators whenever their members have been admitted to a care facility so that NextLevel Health can support the safe discharge of their members.

"Safe hospital to home transitions are contingent on collaboration amongst healthcare providers," says Dr. Cheryl Whitaker, CEO of NextLevel Health. "This partnership will enable us to strengthen our care coordination efforts so that transition to home and back into the community based primary care loop has a chance to occur."

PatientPing, a Boston-based care coordination platform, will provide NextLevel Health members with Pings, real-time notifications whenever one of their members receives care at any ED or facility within the PatientPing network. These notifications will include important context on the patient, including their updated contact information. PatientPing's network consists of tens of thousands of providers nationwide.

"We're excited that NextLevel Health is joining our growing Midwest community," says Jay Desai, CEO of PatientPing. "As a Medicaid MCO, NextLevel understands the value of identifying rising and high-risk members in real-time to ensure timely engagement and improved connectivity to community-based resources. Additionally, NextLevel Health can leverage our diverse provider network of PCPs & FQHCs to share important clinical context to inform care planning and treatment, ensuring member care is truly coordinated between the plan and provider."

NextLevel Health offers a cloud-based health information technology (HIT) system that allows users to see their health information, health profile, medications, doctor's visits and tests, as well as provides access to health education, including videos and articles. Both NextLevel Health and PatientPing share a commitment to increasing accessibility to quality, affordable healthcare for the communities in which they serve. The innovative partnership between the two health technology organizations signifies an inventive, tech-centered approach to reducing barriers to efficient, cost-effective healthcare.
